Giving information on behalf of other people

When you give us information about a person i.e. on a referral form, the person you are referring must have agreed to let you:

• Make a referral in this format
• Agree to you providing relevant information i.e. name D.O.B. Address
• Disclosing information relevant to the referral

Our policy for children

Our web site is not for the use of children under the age of 13. Anyone who's D.O.B. indicates that they are under 13 will not be able to register. Anybody aged 13 or under should contact Haltons Young Persons Substance Misuse Service for under 19s by calling 01928 580242
